Some guys charge double for 6-9" of what they charge for 3-6"; others charge half again. So, it would look something like:

    3-6 $50
    6-9 $100
    9-12 $150 etc

    3-6 $50
    6-9 $75
    9-12 $100 etc

Be careful of your increments, though. In the first example, what would you charge for 6"? Most will do something like:

    3 - 6
    over 6 - 9
    over 9 - 12
